CPPQ – Community Pharmacy Patient Questionnaire - Patient Survey

All pharmacies are required to conduct an annual community pharmacy patient questionnaire (CPPQ), also referred to as the Patient Satisfaction Questionnaire or Patient Survey. The questionnaire allows patients to provide valuable feedback to community pharmacies on the services they provide.

At Allchin Pharmacy, we always try to provide you with the best possible service. We are asking NHS patients to complete a short and anonymous questionnaire about the service we provide.

Click here to complete the questionnaire online
Click here to download the Questionnaire in paper form. Once downloaded and completed, you can post the form to us or bring it in person.

We have Patient Questionnaires forms available in our pharmacy; please ask a member of the staff.

We will use the results of this survey to improve the services we provide at Allchin Pharmacy.

Thank you for your ongoing support and valuable feedback.

What a great service!!! NHS prescribed my 83-year old father medication that he was allergic to. He took a pill before checking and got a reaction. GP could not care less. But male pharmacist stayed open until we managed to get another medication that would stop his allergy reaction well

I've been going for years. I find the key pharmacist Miren so capable, efficient and pleasant. He has been working especially tirelessly through this challenging COVID time. To be saluted. Well done. He is very helpful and well informed.

Directed here by my niece. Wonderful! Friendly staff in a cosy pharmacy packed with everything you could need. (Especially helpful if you're in the market for baby items.) Also used Medic Spot in the store. It's a simple pop-in and see a doctor online for
